New Classified Employees

As a classified employee, you will be required to have a completed personnel file. The Personnel Department will contact you to set up an appointment with our office and Payroll to complete your employment paperwork.

Once your appointment has been scheduled with the Personnel Department, please download and complete the forms listed below.  If you do not have access to a printer, the forms will be available at your appointment. In addition to the forms we will need the following information:

Two forms of original ID- For example, a Driver's License and Social Security Card, a Driver's License and a Birth Certificate, or a Passport are acceptable forms.  Photocopies will not be accepted. 

Voided check or bank letter with your account information for payroll. Payroll will need a copy of your Social Security Card.

Current BCI and FBI background check- If you have completed a BCI and FBI check within the past 1 year, we will accept copies.  If you have not been fingerprinted within the past 1 year, we can fingerprint you at the time of your appointment. There is a $47.25 fee for both the BCI and FBI checks.  We accept cash, check, credit card (Visa, Master Card, or Discover), and money order.
Valid Ohio Educational Aide Permit - If you have a valid Ohio Aide Permit, we must have a copy in our office.  If you do not have an Aide Permit, click here for Step 1 on how to create your OH | ID.  After your OH | ID has been created, click here for Step 2 for a step-by-step guide on how to apply for an Aide Permit. 

**Before we can approve your permit, our office will need documentation of a high school diploma or equivalent. **

ID Badge will be issued 

Mandatory Enrollment in the Public School Works Online Training Program- Effective 2011, any new employee is required to complete the following: Bloodborne Pathogens, Hazardous Communication, Sexual Harassment, Accident/Exposure Reporting, and Safety/Violence Prevention as well as department specific courses.  These training's must be completed within 30 days of the date of registration.  Our office will register you during your appointment using your ESC e-mail address.
