Welcome to the Medina County School's ESC!
As a Certified Medina County ESC Employee, you will be required to have a completed personnel file.
Listed below are all of the necessary forms/documents that are needed for your file. Once these forms are downloaded, printed, and completed, please call our office at 330-723-6393 Ext. 127 or 140 to schedule and appointment.
1. Search Soft Online Application
2. I-9 Employment Verification Form - To complete this requirement, you must come to the Personnel office with identification. A passport alone is sufficient identification for an I-9 Employment Verification Form. If you do not have a passport, you must have a driver’s license and a social security card or a driver’s license and an original birth certificate. Photocopies of these documents will not be accepted.
3. Declaration of Material Assistance/Non Assistance Form
4. A Valid Ohio Teaching License
5. Three letters of recommendation and/or a list of professional references and contact numbers as requested by the Administration.
6. Mandatory Training Using Public School Works - Effective 2011, any new substitute teacher is required to complete the following: Bloodborne Pathogens, Hazardous Communication, Sexual Harassment, Accident/Exposure Reporting, and Safety/Violence Prevention. These training's must be completed within 30 days of the date of registration. Our office will register you during your appointment using your e-mail address.
7. A Criminal Record Check - If you have completed a BCI and FBI check within the past 1 year, we will accept copies. If you have not been fingerprinted within the past 1 year, MCSESC offers fingerprinting by appointment only using the Web Check electronic fingerprinting system for the BCI and FBI checks. There is a $30 fee for the BCI and $30 fee for the FBI fingerprinting. We accept cash, check, credit card (Visa, or Master Card), and money order.
8. ID Badge-Completed at our office during your appointment.
